Puentes para la vida (Bridges for Life) is a new project established by the Lake County government in December 2006 to improve the health and well-being of the Hispanic community in Lake County, Florida. The Rural Women's Health Project was contracted to facilitate Puentes para la vida's initial lay-health worker training program on chronic illnesses. Puentes para la vida focuses on health education, self-help and community unity. The backbone of the project is the development of a community-health worker program. Project Outcomes:
