Puentes para la vida (Bridges for Life) is a new project established by the Lake County government in December 2006 to improve the health and well-being of the Hispanic community in Lake County, Florida.  The Rural Women's Health Project was contracted to facilitate Puentes para la vida's initial lay-health worker training program on chronic illnesses. Puentes para la vida focuses on health education, self-help and community unity. The backbone of the project is the development of a community-health worker program.

Project Outcomes:

  1. Produced La Guia, a Spanish-language resource guide of La Guiahealth and community services in Lake County 
  2. Developed a promotional campaign for Puentes, introducing the program to the Hispanic community and the community at large
  3. Implemented The Lake County Hispanic Health Assessment, surveying over 250 Hispanic community members to document their health needs
  4. Designed, Reflections Magazine, a bi-lingual report of survey findings
